Participants with Annuity account balances over $1,000.00, must download and complete the following forms and must mail back to the Fund office for processing.

•  - Election or Rejection of Direct Rollover
•  - Beneficiary Designation Form
•  - Spouses Qualified Pre-Retirement Notice
•  - Spouses Qualified Joint and Survivor Notice
•  - Special Tax Notice
